Dr. Samara Crudup is a physiatrist specializing in post-acute care, transitional medicine, and functional restoration.
She is a licensed physician with specialized training in peptide and cellular medicine through the International Peptide Society, integrating advanced approaches to support healing and recovery. She completed her residency at Schwab Rehabilitation Hospital, a University of Chicago affiliate program.
For over 16 years, her work as a Physical Medicine and Rehabilitation physician has focused on subacute rehabilitation and interdisciplinary care coordination. Her practice centers on ensuring safe, well-supported transitions home for geriatric patients and individuals managing chronic illness.
Gericare Physiatry was created to support patients and families through the critical transition from subacute rehabilitation to home, reducing complications, providing guidance and advocacy, and promoting healing through innovative cellular medicine approaches.
Our Mission
Transitions home are one of the most vulnerable moments in a patient’s healthcare journey.
